Indiana Family and Social Services Administration (FSSA) Resource Guide
The FSSA resource guide is designed to help connect providers and community- and-faith-based organizations to information about programs provided and administered by FSSA. The resource guide provides information on accessing services in each of the following categories: Early Care & Education, Employment, Financial Services, Food / Nutrition, Health Coverage, Home- & Community-Based Services for the Aged, Blind and Disabled, Mental Health / Addictions, Transportation, and Housing.

MENTAL HEALTH
Mental Health America of Hendricks County has created a comprehensive guide of Mental Health and Wellness resources in or around Hendricks County. Click HERE for access to the guide.
Our Mental Wellness Work Group has created two flyers for distribution within the county to help raise awareness of the state of mental wellness in the county (here), and another aimed at encouraging those with suicidal thoughts to reach out for help through the National Suicide Prevention Lifeline (here). HEALTH/MEDICAL CARE ACCESS SERVICES
Looking for a primary care provider? The Hendricks County Health Partnership’s Accessing and Utilizing Health Care Work Group has created a Primary Care Provider Resource Guide. Click HERE for access to the guide.
Where should you go? It can sometimes be difficult to determine if you should go to an emergency department or an urgent care or to your primary care provider in the event of a health concern. This flyer can help with this decision.
Hope Health Care strives to serve as a primary care medical home for Hendricks County residents without health insurance.
Hendricks County Health Department – Nursing Division
Provides immunizations by appointment to children who are uninsured, on Medicaid, Native American or Alaskan Native, their insurance does not cover vaccines, or privately insured. Flu shots are available seasonally. Adult vaccines are available to uninsured, Medicare, privately-insured, and to those whose insurance does not cover vaccines. TB, hemoglobin, and lead testing available by appointment. HIV/STD testing every third Thursday of the month. For more information, call (317) 745-9222.
Covering Kids and Families is a statewide organization committed to ensuring that all children and families eligible for Medicaid programs are enrolled.
Navigators can assist residents in determining their eligibility for Medicaid and HIP coverage, and provide application assistance for residents using the Federal Health Insurance Marketplace.
Hoosier Healthwise (HHW) is the State of Indiana’s health care program for children, pregnant women, and families with low income.
HIP is an affordable health insurance program from the State of Indiana for qualified uninsured adults.
Medicaid provides health care coverage to nearly 1 million Indiana residents, including eligible low-income adults, children, pregnant women, elderly adults, and people with disabilities.
Medicare is a federal health insurance program for adults age 65 and older and for people under age 65 with qualifying disabilities.
Federal Health Insurance Marketplace
The Marketplace helps people without health coverage enroll in a health insurance plan or other health care coverage based on eligibility.
The Indiana Hospital Association has created this website to allow Indiana residents to compare hospital charges based on procedures and quality ratings.

CHILD AND FAMILY SUPPORT SERVICES
Community Partners for Child Safety – Children’s Bureau
Community Partners for Child Safety is a statewide child abuse prevention program. This service builds community supports for families who self identify or who are referred by community agencies. These families are referred to services that help strengthen their families and prevent child abuse and neglect.
This program offers an in-home visitation program, Community Liaisons (case managers) to assist families in developing and meeting specific family-centered goals, and connections with local community supports and services. Families with children 0-17 that are not actively involved with the Department of Child Services or Healthy Families is eligible for the program.

Family Promise of Hendricks County
The mission of Family Promise of Hendricks County is to fight homelessness in Hendricks County by coordinating immediate shelter and needs assistance programs alongside longer term compassionate support services for families in all areas of life. This programs providing housing, meals, social services, and transportation to participating families. If you are referring a family to the program, please review the Referral Guidelines before referring. To refer a family or find out more about the program call (317) 296-3742.
Sheltering Wings Center for Women
Sheltering Wings provides safe, 24-hour haven for women and their dependent children suffering the effects of domestic violence and helps victims build stable and independent lives through case management and mentoring programs. Help is available 24/7 by calling their hotline at (317) 745-1496.
Hoosier Veterans Assistance Foundation of Indiana
HVAF of Indiana provides housing and reintegration services to veterans experiencing homelessness, and administers programs and services to prevent at risk veterans from becoming homeless. For more information, call (317) 951-0688.
TOBACCO PREVENTION AND CESSATION RESOURCES
Indiana Tobacco Quitline – General Public
The Indiana Tobacco Quitline is a free phone, web, and text-based counseling service for teens and adults in Indiana who are trying to quit smoking or using tobacco. Residents using the Quitline will receive four one-on-one quit coaching sessions (10 for pregnant women) with a trained quit coach, and two weeks of free nicotine replacement therapy (NRT) is available for residents on Medicare, Medicaid, or who are uninsured. The Quitline is available 7 days a week in more than 170 languages. To enroll or for more information, call 1-800-QUIT-NOW or visit their website at www.quitnowindiana.com.
Indiana Tobacco Quitline – Health Care Providers and Community Agencies
Health care providers and community agencies can enroll online for free in the Indiana Tobacco Quitline’s Preferred Provider Program. This program gives health care providers and community agencies proven, professional resources to help patients and clients quit tobacco. Preferred Providers will receive a referral kit in the mail with information about the Quitline and have the ability to fax refer patients and clients to the Quitline, receive status reports on fax-referred patients, receive ongoing communications on the latest research and tobacco control news in Indiana, have access to cessation specialists through the Indiana State Department of Health’s Tobacco Prevention and Cessation Commission, and order materials for the Quitline at no cost.
Indiana Tobacco Quitline – Employers
Employers can enroll online for free in the Indiana Tobacco Quitline’s Preferred Employer Program. This program is a great addition to an employer wellness program and provides them with the resources they need to help employees quit tobacco. Employers enrolled in this program will receive a referral kit in the mail and have the ability to fax refer employees to the Quitline, order materials to educate employees on tobacco cessation, receive updates on new resources and research, and receive monthly aggregate reports on the number of employees participating in the Quitline.
BABY & ME – Tobacco Free at the Hendricks County Health Department
BABY & ME – Tobacco Free is a free quit smoking program uniquely created for pregnant women to help them quit during pregnancy and stay quit after giving birth. During pregnancy, participants receive four quit smoking sessions and referral to the Indiana Tobacco Quitline. After birth, participants receive monthly testing for tobacco use and receive a $25 voucher for diapers for every month up to 12 months they are tobacco-free. Participants can enroll with a Support Partner who completes the program with them and receives an additional $25 voucher.
There are no income or age restrictions for the program. Participants and Support Partners must be a daily smoker or was a daily smoker within three months of becoming pregnant, be willing to quit all tobacco products (including e-cigarettes and chewing tobacco), participate in all quit smoking sessions prior to birth, and take monthly breath tests to determine tobacco use. Promoting Smokefree Pregnancies in Indiana
Promoting Smokefree Pregnancies in Indiana (PSPI) is a statewide, voluntary multi-sector coalition of individuals and organizations dedicated to reducing the prevalence of smoking among women of childbearing age. PSPI provides fact sheets, tool kits, conference calls and other networking events, and various other resources. Visit their website for more information.
Labor of Love is a product of the Indiana State Department of Health in cooperation with other organizations to combat the unacceptable rates of infant mortality in Indiana. The website includes information on tobacco cessation for pregnant women and their families.
